Brain Injury Needs Assessment Survey

Help inform the 2020 TABI State Plan! We want to learn about successes and barriers for brain injury services in Alaska. We also want to identify gaps in services.

We want to hear from

Adults with brain injury,
Family members/caregivers for individuals with brain injury, and
Medical and Service Providers.

The online survey will take 10-15 minutes to complete. You will have the opportunity to win one of sixty $15 Amazon gift cards. Responses are confidential.
