Description: This webinar will discuss the importance of school and community partnerships for diabetes prevention in youth. It will cover ways to practically treat obesity and prevent T2DM in children. Lastly, this webinar will discuss the components of an evidence-based healthy lifestyle program and explore potential ways to share resources among the community to make such a program available to primary care clinicians.

Topic: Creating YOUR OWN Diabetes Resource Toolbox!
Description: The National Center for Farmworker Heath (NCFH) invites you to participate in a webinar where you will be able to gather and create your very own Diabetes Resource Toolbox. Presenters will describe and feature a variety of culturally appropriate Diabetes Prevention and Self-Management resources and tools from NCFH’s Diabetes Resource Hub curated especially for Migrant and Community Health Centers. Presenters will also preview additional diabetes related resources and tools from affiliate partner web pages including the American Diabetes Association (ADA), the Association of Diabetes Care and Education Specialists (ADCES), and the Special and Vulnerable Populations Diabetes Task Force. Participants will receive guidance on identifying the most appropriate resources and tools they should include in their toolboxes based on their patient populations and diabetes program efforts. Participants will also discuss ways to use their Diabetes Resource Toolboxes and how best to integrate these resources and tools into their current service delivery practices.
